To apply to foster for AAN, please complete the form below. This form is designed to provoke thought about whether or not you are prepared to offer the best possible experience to an animal in need. We expect several things of our fosters, including compassion, perseverance and reliability. Do you exemplify these qualities? Please give each question serious thought as it will help us to determine your eligibility. Sharing your home with an animal, even temporarily, is a responsibility to be treated with care and respect. Many of our animals will require extra loving care as they recover from illness or learn correct behavior. If you are up to the challenge and ready to reap the rewards of helping to save the life of an animal, apply today!

Please note: All health care of fostered animals will be provided by AAN, unless otherwise stated and agreed to by both AAN and the foster. Please contact AAN for an approved veterinarian.

 

 

Do you feel capable of handling and help to mitigate behavioral problems like "marking in the house," separation anxiety or fearfulness with or without a veterinarian or behaviorist's assistance?

 

 

Can you describe any obedience training experience, or past personal experience with behavioral or medical issues?

Please provide AAN with three personal references with telephone numbers. Only one of the three may be a family member.

 

 

 

 

Are you willing to make an attempt to work through unexpected hardships with your foster pet within reason, i.e., housetraining, separation anxiety, moderate unruliness, fear, and acclimation to other pets, etc?

 

 

Do you agree to contact AAN and only AAN in the event that an animal must be removed from your home?

 

I agree and understand that should I choose to adopt this animal, I am responsible for the adoption fee in full, as outlined on the adoption application, unless otherwise stated and agreed to by a AAN official.

 

 

Foster Contract and Affirmation

 

I, (name) ________________________________, am applying to foster the following animal, _______________________________.

 

In the case of fostering, the person(s) agreeing to foster an animal (the foster family) will follow all relevant procedures and guidelines regarding the animal as stated in the AAN’s Contract. If the animal cannot be kept by the foster family, the foster family shall notify a AAN Coordinator immediately. The animal shall not be sold, abandoned, relinquished to a pound or shelter or to a Rescue group other than AAN, transferred, or given to any individual, or to any retail or wholesale establishment. We appreciate your desire to help AAN by fostering one of our adoptable pets, but in order to ensure the safety of your foster we ask that you review and sign the following terms and conditions.

1. If for any reason you have to return your foster you must return him/her to a AAN representative. An AAN representative will be more than happy to pick up your foster if you cannot keep them but we may require 5 business days to make other arrangements.

2. AAN must approve all adoptions; you are not to give your foster to anyone without the approval of a AAN representative.

3. Fosters are to be kept ON LEASH AT ALL TIMES!! There are no exceptions.

4. If you must take your foster to the vet we ask that you take them to the AAN vet (unless it is an emergency). Or unless other arrangements have been made and agreed to by both AAN and the foster.

5. In signing this foster agreement with AAN, I declare that I am aware that: animals are different from human beings in their responses to human actions; the actions of animals are often unpredictable; children and animals must be closely supervised when interacting; an animal’s behavior may change after it leaves the shelter and accustoms itself to its new environment.

6. I am aware that the educated opinions of the shelter staff do not constitute a claim or guarantee as to the temperament, health or mental disposition of any animal available for foster or adoption.

7. I herby accept possession, legal guardianship and responsibility for the animal I am fostering and discharge AAN forever from liability for any injury or damage to any person or property caused in the future by said animal, and from any causes of action, claims, suits, or demands whatsoever that may arise as a result of such injury or damages.

8. All of the information I have provided is true and complete to the best of my knowledge.

9. Should a pet be placed with me it will reside in my home as a pet. I agree to provide the dog with adequate food, water, shelter, affection and medical care which will be paid for by AAN.

 

10. I agree that while fostering and if I decide to adopt this pet he/she will be kept as an indoor pet. This animal will not be chained or tethered outdoors for any length of time. They will live inside with our family.

AAN does not adopt outside dogs or cats, we believe that if you want to adopt a pet they should live inside with you and be part of your family. We do not believe in chaining or tethering an animal, if you would like an animal for this purpose than you should go elsewhere to adopt.

Dogs can be let out to go potty and to play in a fenced yard but should not be left out unsupervised all day or night. We recommend crating your new friend while you are away from home, if this is not possible at this time then the pet should be returned to AAN at once.

WHAT EACH FOSTER SHOULD KNOW

1 Dogs in foster care are to be placed ONLY by an authorized Animal Adoption Network, Inc Representative. While fosters have input on applicants, final applicant approval will be necessary.

2 Foster homes are responsible and liable for the actions of foster dogs while in their care.

3 If unable to continue to foster a dog for any reason, the Foster agrees to keep said dog until another foster home can be found or until the dog can be safely placed into another environment.

Animal Adoption Network, Inc will provide necessary medical expenses for all foster dogs, providing that the foster dogs are brought to a veterinarian approved by the rescue and that the visit was previously approved by a Animal Adoption Network, Inc Representative.

 

Hold Harmless Waiver

I am aware that by signing this application, I agree to provide what is needed for the health and safety of a dog in my care for Animal Adoption Network, Inc. I further agree to assume all responsibility for the foster dog(s) in my care while on or off my premises. I also agree to assume all liability for the dog while I foster it for Animal Adoption Network, Inc.

I fully understand and agree to assume all risks involved in any and all duties that I perform for Animal Adoption Network, Inc in my volunteer capacity and I agree to hold Animal Adoption Network, Inc harmless for any injury(s) that I might sustain during the course of my volunteer duties.

This waiver does include myself, all of my family members and descendants forever from seeking any legal action whatsoever against Animal Adoption Network, Inc or its Representatives.

I also understand that when I resign as a foster to return any equipment and unused supplies provided to me by Animal Adoption Network, Inc.

By signing this application and waiver, I declare I am at least 18 years of age.
